Sliding fee discounted services are offered to anyone who presents for health care services based on eligibility criteria determined by the Federal Poverty Guidelines. Eligibility for sliding fee scale discounts is based on income and household size. Written documents verifying income must be presented as part of the application for sliding fee discounted services.
